Housing Program Name: HOPWA TBRA Program (c/o Louisville Metro Housing and Family Services)

Program Description: Provision of tenant-based rental assistance to qualifying HOPWA clients.

Eligible Populations: Single, 1- and 2-parent families; men and women.

Eligibility Requirements: Homeless, disabled; income must be below 80% of average median income; must be HIV positive.

Referral Process: Through House of Ruth, Inc. (607 E. Catherine Street).

Capacity: 26 clients (56 household members).

 

Housing Program Name: CHI

Program Description: Scattered site housing units; provides case management.

Eligible Populations: Single; men and women; HIV+.

Eligibility Requirements: Homeless, disabled; income must be below 80% of average median income for the area; must be HIV positive.

Referral Process: 8:45 a.m. - 4:45 p.m., phone, walk-in, case manager referral.

Capacity: 2

 

Housing Program Name: CH2

Program Description: Scattered site housing units; provides case management.

Eligible Populations: Single; men and women; HIV+.

Eligibility Requirements: Homeless, disabled; income must be below 80% of average median income for the area; must be HIV positive.

Referral Process: 8:45 a.m. - 4:45 p.m., phone, walk-in, case manager referral.

Housing Program Name: Glade House

Program Description: Permanent supported housing including life skills, recovery, and therapy.

Eligible Populations: Single; men and women; HIV+.

Eligibility Requirements: Homeless, disabled; 30% of income; must be HIV positive.

Referral Process: 8:45 a.m. - 4:45 p.m., phone, walk-in, case manager referral.

Capacity: 10

 

Housing Program Name: Homes with Heart

Program Description: 13 scattered site housing units.

Eligible Populations: Single, 1- and 2-parent families; men and women.

Eligibility Requirements: Homeless, disabled; low income (less than 80% of median for the area); must be HIV positive.

Referral Process: 8:45 a.m. - 4:45 p.m., phone, walk-in, case manager referral.

Program Description: Site-based S+C (scattered site).

Eligible Populations: Single; men and women; must be HIV+.

Eligibility Requirements: Homeless, disabled; 80% of median income; must be HIV positive.

Capacity: 4
