Program Description: SRO rooms for formerly homeless men.

Eligible Populations: Single.

Eligibility Requirements: Homeless, disabled and substance abuse; background check required; no sexual offenders; rent is 1/3 income.

Referral Process: Monday - Wednesday 8am - 1pm by appointment (ind. or case manager).

Capacity: 11 units.

Housing Program Name: Mainstream Program

Program Description: MHA rental assistance + Center for Accessible Living supportive services.

Eligible Populations: Single, 1- and 2-parent families; men and women. Head of household or spouse must be disabled.

Eligibility Requirements: Disabled; criminal record check (Eligible per LMHA Admin Plan); income guidelines apply. Head of household must be 18 y/o or legally emancipated.

Referral Process: Referrals made to Center for Accessible Living Program Coordinator contact: LeeAnn Thomas at 589-6620.  Head of household or spouse must be disabled.

Capacity: 300

   

Housing Program Name: Olmstead Program

Program Description: LMHA rental assistance + referring agency supportive services.

Eligible Populations: Single, 1- and 2-parent families; men and women. Inappropriate level of institutionalization must be primary issue.

Eligibility Requirements: Disabled; criminal record check (Eligible per LMHA Admin Plan); income guidelines apply. Head of household must be 18 y/o or legally emancipated.

Referral Process: Active partner agencies are Center for Accessible Living, Program Coordinator LeeAnn Thomas at 589-6620 and Wellspring, Kathy Dobbins at 637-4361.

Housing Program Name: Partnership for Families Program

Program Description: LMHA rental assistance + CPS supportive services.

Eligible Populations: 1- and 2-parent families; men and women. Housing must be only remaining issue.

Eligibility Requirements: Criminal record check (Eligible per LMHA Admin Plan); income guidelines apply. Head of household=18 y/o or legally emancipated.

Referral Process: Must be active with CPS (Child Protective Services) and housing the last remaining issue.  Contact person/gatekeeper:  Renee Morgan at 595-5751 Ext. 5265 at CPS.

Housing Program Name: Public Housing Assistance

Program Description: Provides low-income families with public housing.

Eligible Populations: 1- and 2-parent families; men and women.

Eligibility Requirements: Homeless; disabled; domestic violence victim; substance abuse; criminal record check - would have to qualify. Income requirements based on 2009 income limits.

Referral Process: Hours of operation are 8:00 a.m. to 4:30 p.m.; applications are taken daily. walk-in interviews are conducted for elderly and disabled applicants eligible for one bedroom units at LMHA high-rise; Walk-in interviews are conducted on Tuesdays, Wednesdays, and Thursdays between 9:00 a.m. and 11:30 a.m.

Housing Program Name: Scholar House

Program Description: LMHA rental assistance + Scholar House supportive services.

Eligible Populations: Single parent; must be pursuing educational goal.

Eligibility Requirements: Criminal Check; eligible per LMHA Admin Plan; income guidelines apply; Head of household = 18 y/o or legally emancipated.

Referral Process: Project Women/Scholar House initiate all referrals.

Capacity: 56

     

Housing Program Name: Section 8 Rental Assistance Program

Program Description: Housing Choice voucher program.

Eligible Populations: 1- and 2-parent families; men and women.

Eligibility Requirements: Criminal Check; eligible per LMHA Admin Plan; income guidelines apply; Head of household = 18 y/o or legally emancipated.

Referral Process: Applications available at 801 Vine Street; must be mailed in.

Capacity: 9400

   

Housing Program Name: Veterans Administration Supportive Housing (VASH)

Program Description: LMHA rental assistance + VA supportive services.

Eligible Populations: 1- and 2-parent families; men and women; must be homeless veteran.

Eligibility Requirements: Criminal Check; eligible per LMHA Admin Plan; income guidelines apply; Head of household = 18 y/o or legally emancipated.

Referral Process: VA initiates all referrals.

Housing Program Name: Villager Program

Program Description: LMHA rental assistance + Center for Women and Families case management.

Eligible Populations: 1- and 2-parent families; men and women; must be eligible per CWF criteria.

Eligibility Requirements: Criminal Check; eligible per LMHA Admin Plan; income guidelines apply; Head of household = 18 y/o or legally emancipated.

Referral Process: Center initiates all referrals.

Program Description: Tenant Based Rental Assistance for Homeless, Disabled individuals/families.

Eligible Populations: Single, 1- and 2-parent families; men and women.

Eligibility Requirements: Homeless, disabled; 80% of average median income requirement; must have case management services.

Referral Process: Referrals are made through a case manager providing services.

Capacity: Approx. 253 households.
