Inappropriate level of institutionalization must be primary issue. Eligibility Requirements: Disabled; criminal record check (Eligible per LMHA Admin Plan); income guidelines apply. Head of household must be 18 y/o or legally emancipated. Housing Program Name: Partnership for Families Program Program Description: LMHA rental assistance + CPS supportive services. Eligible Populations: 1- and 2-parent families; men and women. Housing must be only remaining issue. Eligibility Requirements: Criminal record check (Eligible per LMHA Admin Plan); income guidelines apply. Head of household=18 y/o or legally emancipated. Housing Program Name: Public Housing Assistance Program Description: Provides low-income families with public housing. Eligible Populations: 1- and 2-parent families; men and women. Eligibility Requirements: Homeless; disabled; domestic violence victim; substance abuse; criminal record check - would have to qualify. Income requirements based on 2009 income limits. Referral Process: Hours of operation are 8:00 a.m. to 4:30 p.m.; applications are taken daily. walk-in interviews are conducted for elderly and disabled applicants eligible for one bedroom units at LMHA high-rise; Walk-in interviews are conducted on Tuesdays, Wednesdays, and Thursdays between 9:00 a.m. and 11:30 a.m. Capacity: 4279

Referral Process: Referrals are made through a case manager providing services. Capacity: Approx. 253 households. |
