Housing Program Name: HUD/VASH (VA Supported Housing)
Program Description: Collaborative of HUD&VA: Permanent tenant-based supported housing.
Eligible Populations: VA-eligibility required; single, 1- and 2-parent families; men and women; youth.
Eligibility Requirements: Homeless; must have income; must be amenable to case management.
Referral Process: 8AM-5PM Mon-Friday, Referrals accepted for assessment from HCHV and Grant/PerDiem staff/programs,
Veteran's Outreach; see Louisville Metro Housing Authority for further eligibility requirements.
Capacity: Per voucher distribution-175.

Housing Program Name: Collaborative Housing Initiative (CHI)
Program Description: Permanent supportive housing.
Eligible Populations: Single, 1- and 2-parent families; men and women.
Eligibility Requirements: Homeless; disabled; must be amenable to case mamagement.
Referral Process: Must have received treatment for substance abuse issues, mental illness or both, and committed to maintaining their recovery and permanent housing. Must have a minimum of 6 months sobriety (clean & sober) and/or 6 months of sustained mental health treatment. Must be employed, in school, volunteering or a combination of these for 40 hrs. per week unless a disability precludes them.
Capacity: 9 single adults and 3 family units.

Referral Process: Clients must meet all eligibility requirements for HUD, SHP, and PSH.
Capacity: 30 units.

Program Description: Permanent supportive housing for disabled, homeless women.
Eligible Populations: Single; women.
Eligibility Requirements: Homeless; disabled; if tenant has income, must pay 30% per HUD formula.
