Shelter Plus Care (See below for downloadable forms)
The purpose of the Shelter Plus Care program is to provide permanent housing in connection with supportive services to homeless people with disabilities and their families. The program provides rental assistance for a variety of housing choices, accompanied by a range of supportive services funded by other sources. The program is designed to serve a population that has been traditionally hard to reach - homeless persons with disabilities such as (but not limited to) serious mental illness, chronic substance abuse, and/or AIDS and related diseases. The Shelter Plus Care program was built on the premise that housing and services need to be connected in order to ensure the stability of housing for this population.
To receive Shelter Plus Care you must meet certain qualifications:
● You must be homeless. You must be living in a car, on the street, places not meant for human habitation; an emergency shelter, transitional housing or being discharged from a hospital or other institution with no place to reside. You must be able to provide verification of your homeless status and that you have no support network.
● You must be disabled. A disability must be physical, mental, or emotional impairment which is expected to be of indefinite duration.
● You must be willing to participate in an intensive case management process.
If you believe you are a good candidate for the Shelter Plus Care Program you need to first seek case management services from a partnering agency.

Partnering agencies have all applicable Shelter Plus Care application materials and can begin the program entry process. PLEASE be aware that the Shelter Plus Care program is currently operating on a WAITING LIST for new applicants.
